Terms of Service - Eurostar Soccer Camp

I hereby recognize and acknowledge that my child's participation in EuroStar Soccer Camp may involve bodily injury and/or emotional injury to myself and/or my child. In consideration of my child being permitted to participate in such event, I for myself, my child, my heir, my executors and administrators, hereby voluntarily and knowingly release, waive, and discharge EuroStar Soccer School, its officers and staff from any and all liability. In addition, I agree that my insurance company or I will pay for medical, hospitalization, damage or any other expenses resulting from my child's participation. I give consent for my child to be photographed and or filmed while participating in camp activities and for the resulting material to be used by EuroStar Soccer School for promotional purposes. During a residential soccer camp program, all students are expected to be in their rooms by 11pm. Students are also expected to not enter each others suites unless that suite is their own. Students are to spend their free time in their suites, lounges and or at the computer lab. Failure to obide by any of the above rules will result in the student automatically and without warning being sent home without a refund.

Refund Policy
Withdrawing anytime before the camp, you receive a full refund. Withdrawing between the first day and half of the program you receive a 20% refund. Withdrawing after attending half of the program you receive no refund.

Terms of Service - Eurostar Football Academy

Code of Conduct
PLAYER's code of conduct:
Show respect toward:
- one-self, the academy, the game, team-mates, parents, opponents, officials, coaches and their decisions,
laws of the game, the training grounds.
Must be punctual to all practices and games, and any other activity planned for the team.
Practices: players must be dressed and ready for practice 5 min. prior to start time
Games: players must be dressed and ready to play 45min. prior to scheduled kickoff time
EFA Players are not to participate on any non EFA Futbol Team Programs or any other soccer events
without the express written consent of the Academy Director. Failure to respect this code will result in
immediate release from EFA Academy Team Programs.
Injuries caused by activities outside the academy could result in possible dismissal.
EFA players on U14 teams and older are not to participate in activities that interfere with the EFA
Academy Team Programs schedule. Please note that the schedule is subject to change at any time.

EFA players on the U15 Team Boys programs and older will only be substituted once per game.
Must be prepared at all times, with correct attire and equipment for all practices and games.
Must have discipline on and off the field, show consistent attendance at practices, games and training cycles
planned for the team.
Players must have commitment to their team and are expected to participate in all activities planned for
their team and interact with teamates.
Show loyalty to the Academy, team and its members.
Must follow proper nutrition and place full effort into the mandatory at home fitness and technical skills training programs.
Players should always show desire, determination and positive attitude toward their learning process and show focus
during training sessions and games in order to achieve the developmental expectation planned.
Players are expected to use their individual skills to help the team achieve success, while at the same time, contribute as a team player.
Players must recognize that they are ambassadors of the Academy, and be good role models by exhibiting exemplary behavior at all times.
Players win games without boasting, lose without excuses and never quit.
Have fun while producing 100% effort at all times.

CONTRAVENTION OF ANY OF THE ABOVE WILL AFFECT PLAYING TIME.
PARENT'S code of conduct:
Must respect the coaches and their decisions, and should show respect towards the laws of the game, the players, officials and opponents.
Parents, under no circumstances, are allowed to coach, or deliver any type of instruction other than cheering for their child or for the team.
Parents will give positive reinforcement, and always encourage their child and the team.
Parents are not allowed to argue with officials, nor show any disrespect for the official's judgment, under any circumstance.
Parents must not use foul language towards any member of the team, coaches, officials, or members of the opposition.
Must refrain from discussing game related issues with the coaches (i.e.: strategies, tactics, playing time).
Coaches will only discuss player development with parents.
Parents must ensure the punctual arrival of their child to all practices and games.
Parents will also keep coaches and managers informed on any absences.
Parents are encouraged to maintain good relations with members of the EFA Academy Team Programs.
Parents are encouraged to use the proper channels of communication (i.e. contact the Academy Director)
if a need, concern or suggestion arises.
Parents must try to control negative emotions and are encouraged to think positively.

Medical Waiver
Eurostar Football Academy Assumes no responsibility for any injury, which your child may incur during his/her participation
during the training sessions. In the event that an injury requiring medical assistance does occur, you authorize the coach or
company official to allow whatever medical attention is required from a doctor or qualified
medical personnel and waive any liability against said coach or team official and Eurostar soccer and its members.

Media Release
I hereby grant Eurostar Football Academy or any of its agents, the right and permission, in respect of the photographs
and video which Eurostar Football Academy or its agents have taken of me or my child in which I / We may be included
with others, to copyright the same in its own name or otherwise; to use, refuse, publish and republish in the same in
whole or in part in conjunction with any printed matter in any and all media now or hereafter.

Refund Policy
Non Attendence by a participant does not constitute a notice of withdrawal. Request for refunds must be submitted in
writing to the Academy Director prior to the start of session. A $50.00 administration fee will be applied to all refunds.
A $20 fee will be charged for all NSF Cheques.
